Required paperwork
Apprentice Teachers must complete all of the following and turn them in to CCFS office in West House before starting work with children. Documents saved as pdfs or MS Word may be uploaded (no screenshots/photographs please) to a secure Box upload site. Before uploading, please rename documents appropriately (yourname-documentname, ex. KaraSmall-LiveScan). Pdfs may be combined into one document before uploading, ex. KaraSmall-alldocs).
- LiveScan (fingerprinting) form New staff complete and print form which they must take with official ID to PDQ Fingerprinting (216 F Street Davis). Walk in without an appointment, but check for open hours. The signed copy of the 1st page must be returned to CCFS office as a receipt.

- Online training: Integrated Pest Management. The California Healthy Schools Act requires that childcare and school staff take one Integrated Pest Management class every year. Create an account at https://online2.cce.csus.edu/dpr/login/index.php and complete the Basic Integrated Germ Management course; save and turn in certification of completion.

Online training: Mandated Reporter Training-General and Child Care Modules ; 4 hours https://mandatedreporterca.com/training; you must register first to obtain a certificate of completion. If previously completed, provide documentation. Must be repeated annually

You must have these health records on file in the office - please submit if not already on file from previous quarter enrolled in HDE140L. (CA immunization records my now be obtained online here)
- TB Test: Employees must have a new TB test completed or submit proof of negative TB test less than 12 months old (or positive test with clear chest xray less than 4 years old). Must show date and result. Any student can be tested at the Student Health and Counseling Services center. Must show student's full name on the document. Alternatively, a health professional can complete the Risk Assessment and waiver form and you may submit pages 1-2.
- Covid-19 vaccination(s) in alignment with campus directives
- Current Flu season vaccination in line with campus directives (must show date between August 1 and December 31 of current school year) and full name on the document.
- Pertussis vaccination documentation (usually shown as DTP or DTaP) - must show full name and the dates of immunizations
- Measles vaccination documentation (usually shown as MMR) - must show full name and the dates of immunizations
