Hello and Welcome to the Early Childhood Laboratory (ECL), Center for Child and Family Studies (CCFS) at UC Davis!
Before starting work, new staff must complete all of the following and turn them in to CCFS office in West House. Documents saved as pdfs or MS Word may be uploaded (no screenshots/photographs) to a secure Box site. Before uploading, please rename documents appropriately (yourname-documentname, ex. KaraSmall-LiveScan). Pdfs may be combined into one document before uploading, ex. KaraSmall-alldocs).
Checklist
- New staff complete and print form which they may get done free at PDQ Fingerprinting (216 F Street Davis). You may walk in without an appointment, but check for open hours. Bring an official picture ID. The signed copy of the form must be returned to CCFS office. If you do decide to get the LiveScan elsewhere, please bring back an itemized receipt, and sign and date it.
- Contact your college(s) to have original transcripts sent to: CCFS, UC Davis, One Shields Ave., Davis, CA 95616-5270
- Photocopy all post-high school diplomas
- Bring or send documentation of all relevant ECE coursework outside your degrees - copies are acceptable
Required forms:

Health Screening - to be completed by physician documenting checkup less than 12 months old.
Required training:
- Complete the General Mandated Reporter Training https://mandatedreporterca.com; 4 hours. You must first register for an account, take the training and obtain a certificate of completion. If previously completed, provide documentation. This is a one time only training.
- Next, complete the Childcare Providers module of the Mandated Reporter training (3 hours). This must be repeated every two years. If you have documentation of this training less than two year old, please provide it.
- Integrated Pest Management online training. The California Healthy Schools Act requires that childcare and school staff take one Integrated Pest Management class every year. Create an account at https://online2.cce.csus.edu/dpr and complete one of the following each year. Save and turn in certification of completion.
- Child Care First Aid and CPR, infant, child and adult. The CCFS has an all-staff training on-site every other September.
Immunizations:
- TB Test: Employees must have a new TB test completed or submit proof of negative TB test less than 12 months old (or positive test with clear chest x-ray less than 4 years old). Must show name, date read and result.
- COVID-19 vaccinations documentation (initial and booster)
- Influenza vaccination documentation (dated between August 1 and December 1 of current school year)
- Pertussis vaccination documentation
- Measles vaccination documentation
